If you have hired or plan to hire a caregiver for a child or for an elderly or disabled person and if you can receive the caregiver's full cooperation and written permission, you can obtain an FBI FINGERPRINT CHECK of this caregiver.

Based upon the fingerprints submitted to the Federal Bureau of Investigation, if a criminal history record is found or not found, the FBI will confirm its existence or provide a copy of the "rap sheet" (criminal record) document or a copy of the "no record found" document directly to you or anyone including the subject provided that the subject gives his NOTARIZED written permission on the following Authorization Form, signs the following Cover Letter Form as the Requestor...Remember that the Requestor is the Applicant who is providing his fingerprints. Obtain an official FD-258 FBI Fingerprint Card or download a copy from the FBI and have the subject's fingerprints taken at any local law enforcement agency. By US mail, send the completed Fingerprint Card (filling in required information) along with a certified check, money order, or Credit Card payment( using the Credit Card Form Click Here for Forms. (On the Credit Card Form, remember that the Customer or Requestor refers to the person providing his fingerprints) in the amount of $18.00, made payable to the Treasury of the United States,and the Authorization Form, the Cover Letter Form from the subject requesting the record for PERSONAL REVIEW and the Credit Card Form (if applicable),to:
